Pathways Home is aimed at preventing homelessness and supporting those in our community who are vulnerable and at risk. Stonepillow is looking to recruit applicants with experience of supporting people with varying degrees of support needs. You will be working closely with Chichester District Council, and the Supported Accommodation Team within Stonepillow.

This role involves visiting clients in their own homes or at community locations within the Chichester District area covering from Petworth to Selsey. You will be responsible for coordinating interventions to ensure that the right support is provided at the right time or signposting on issues like housing options, tenancy issues (e.g. disrepair or dampness), housing benefits and welfare support, debt, setting up utilities, linking into community services or employment.

Your role is to support vulnerable single people and families who may be experiencing a number of complexities including mental health and/or substance misuse issues or who may be in some other way socially disadvantaged. This role centers on prevention before they face possible homelessness.

The aim of the role is to support and signpost clients into services that will improve their housing, health and wellbeing while improving the client’s quality of life. This is achieved through the provision of engagement and individualised support plans, targeted interventions, signposting and group work, working closely with other projects within Stonepillow to identify a move on pathway, as well as external agencies.

Main Duties and Responsibilities

  • Lead on providing a floating support service to clients within their own accommodation or within the community, using a person-centered model and Trauma-Informed Care approach, work directly with those in our community in a positive, persistent, proactive and assertive way to secure meaningful engagement to bring about sustained outcomes for them.
  • Assist clients in a friendly and compassionate manner understanding their individual needs and ensuring we deliver functional services to them.
  • Take a proactive approach towards compassion and case management, working with the identified cohort and other agencies to design & deliver effective interventions that create sustained pathways to prevent homelessness and increase engagement with community services.
  • To lead, co-ordinate and manage a caseload of 25 open cases at any given time with an even split of low, medium and high support needs.

In order to be successful in this role you will need:

  • Experience of working with clients who present with challenging behaviour's.
  • Good knowledge of alcohol, drugs and issues around addiction and relapse management
  • Good knowledge of mental health, personality disorders and dual diagnosis
  • Experience of working with people who are or are at risk of becoming street homeless.
  • Practical knowledge and understanding of the drivers of homelessness and rough sleeping
  • The ability to forge effective working relationships within a team
  • Excellent computer literacy skills and ability to make effective use of systems
