Adult Services Hospital Team. 1 year temporary/secondment opportunity. This post forms part of the new `Closer Care to Home Project', which aims to reduce admissions to St Richards Hospital. You will both help prevent inappropriate admission in A&E and co-ordinate discharge to the community. Experience of working in a hospital and multidisciplinary team is therefore preferable. The role will involve assessment, care management and multi-disciplinary work. You'll need an understanding of safeguarding adults and be able to work creatively with the use of initiative. The ability to work as part of a multi-disciplinary team and manage a busy workload is essential.
