We have a vacancy in our Hospital Services and want to recruit an enthusiastic individual who will enjoy working with older people and can support us to continue developing and delivering a quality service.

Take home and Settle Coordinator,
15 hours per week, over a 3 day rota Monday to Saturday, Salary £7,620
Based in St Richards Hospital,

The team of Take Home and Settle Co-ordinators provide vulnerable people with the support they need to return home safely rather than be admitted to hospital due to a lack of support at home.
our Clients are people who live alone and do not have immediate support at home.
We collect patients from the discharge lounge:
•take them home,
•maybe do a bit of shopping on the way,
•settle them at home checking heating and other facilities are working
•ensure they have eaten
•make sure they are comfortable
•notify network of support they are home
The successful applicant will need a practical approach, good people skills and will require a full driving licence and use of a car.
