Age UK West Sussex delivers services and activities to support vulnerable people, their carer’s and families. Our aim is to promote well-being and make later life a fulfilling and enjoyable experience.

We work with West Sussex County Council Prevention Assessment Team to support the wellbeing of adults and older people and have the following vacancies.

This role will provide access to practical support to vulnerable people where a specific need exists. The purpose is to engage directly with clients to provide one-off, time limited practical support to access sources of support available within the local area. The successful applicant will be part of a multi-disciplinary team, delivering a range of expertise, including health and social care to enable people to make any changes in their lives or make informed choices necessary to resolve their issues.

You will need to have a full driving licence and use of a car.
