An exciting opportunity has arisen for an Assistant Care Manager to join one of the following teams:

Littlehampton Community Team – based at Centenary House, Worthing
(1 x 37 hours per week, 1 x 30 hours per week)
Shoreham Community Team – based at Glebelands, Shoreham by Sea
(1 x 37 hours per week)
Prevention Assessment Team Southern – based at Glebelands, Shoreham by Sea
(1 x 37 hours per week)

The positions are available on a full and part time basis. Working hours and length of contract will be discussed at the interview.
Under guidance and regular supervision, you will provide a social work support service to those people who access or need the service to include assessment, planning and review of care packages. You will be expected to work within organisational, policy and legislative requirements.

You will demonstrate compassion and empathy in your approach to working with adults with social care needs and should have relevant experience in the care sector. You will work individually under supervision from a qualified member of staff, and as part of a team to meet with individuals and carers to assess needs, formulate outcomes and undertake reviews using a person-centred approach designed to provide meaningful choice and promote wellbeing.
In addition to your community role you will be required to be part of the staff rota for the countywide team who screen and triage referrals for adults to determine customer eligibility for an adult service. This requires working in Horsham during the period of rotation, which is approximately 1 week in 4. Appropriate training will be provided for this and the role is currently subject to a review.
Progression to Assistant Care Manager – Grade 7 will be through completion of at least 12 months service at Assistant Care Manager – Grade 6 and a formal assessment by the Team Manager that evidences the ability to work at the higher grade.
