We are looking to appoint a Team Leader to lead and manage a county-wide team of specialist Carer Assessment Wellbeing Support Workers to provide a consistent information, advice, assessment and support service for family and friend carers, ensuring CSWS performance targets are achieved within budget.

Our teams offer support to adults and young adults who are unpaid family and friend Carers. The charity has been accredited as an ‘excellent’ local provider of quality support for Carer wellbeing. Team Leadership, you will manage the specialist county-wide team to deliver best value, high quality, statutory carer assessments working closely with other Team and Programme Leads and Managers to ensure all front-line Carers Support staff are trained and developed to consistently offer carers high quality support. Service Management You will ensure carers are offered a choice of personalised and time-focused support to best meet their needs and choice including 1:1 casework, groups, telephone/web-based support. You will Integrate support with other services and resources, internal and external. Ensure a direct link for carers to specialist support and social care and health support services across localities so that carers experience joined up local support. You will introduce service changes and improvements to widen the carer assessment offer, reduce waiting times, increase the number of carers receiving a service and enhance carer experience. Actively review waiting lists and reassess priority, keeping in touch with carers. The ideal candidate will have experience of social work and social care IT infrastructure, leading a team across disparate locations, caseload management ,delivering person-centred outcomes within a caring service and related quality assurance processes.
