The Community Learning Disability Team – Transition is designed to focus on working with young people and adults with a learning disability through their transition to adult social care services. The focus is on starting transition work early in order to ensure this often stressful process can be managed smoothly.

The CLDT-Transition meets the statutory duties of the Council by undertaking assessments of need, support planning, monitoring and reviewing of customers with eligible social care needs.

The team comprises of social workers and care managers who work closely with health colleagues within the four integrated Community Learning Disability Teams.

The team covers the whole of West Sussex and out of area services for people who are the responsibility of West Sussex County Council, and team members will be located in each of the Community Learning Disability Teams.

The aim of the team is to facilitate a smooth transition into adult social care services.

You will provide an accountable statutory social work service to those people who access or need the service to include assessment, planning, monitoring and review.

You will be expected to work within organisational, policy and legislative requirements.
