There is an exciting opportunity for a short term contract into the role of Social Worker in the Older Peoples Mental Health Team in the north reporting to the team manager. This is a well-established team consisting of 1 senior social work practitioner, 9 social workers and 2 assistant care managers. We work with customers with both functional and organic mental health difficulties in the community and acute inpatient wards. The team specialises in the most challenging areas within mental health therefore a good knowledge of mental health, statutory frameworks including Mental Health Act and DOLS is desirable. Four team members are Best Interest Assessors and 2 are approved Mental Health Professionals.

The successful candidate will be working with colleagues in Sussex Partnership Trust including Living well with dementia teams, the Memory assessment teams and the Dementia Crisis team, as well as the functional mental health teams. As the team covers the North, you will need to be supportive to the local workers who are based over three sites, Crawley Mid Sussex and Horsham. The role will include attending ward rounds, under taking assessments of need, reviews, hospital discharges from both functional and the dementia care units, as well as links with the acute hospital settings. You will also need a good understanding of S117 funding, continuing health care and the interrelationship between adult health care funding and health.

This short term contract is an opportunity until April 2017 and is an exciting challenge in the field of dementia, where we have Crawley Dementia Friendly Town with the other areas also now exploring developing a Dementia friendly strategy with the residents.
