Do you want to take up an exciting opportunity to work with vulnerable adults in Horsham or Mid-Sussex? As an Assistant Care Manager, you will make assessments and reviews of need for social care support, and attend relevant meetings.

You will have a positive approach to support people towards independence, making use of community resources. You will have good record keeping skills with experience of using IT systems. A team player, you will participate in the Duty system and work with colleagues from other fields such as Health. You will be expected to manage your own caseload with regular supervision to support you.
