Would you like to be part of the most significant change for children and young people with Special Educational Needs and Disabilities and their families in the last 30 years?

We need full-time and part-time Planning Coordinators to work and support families through the Education, Health and Care Needs Assessment and planning process, including the offer of a personal budget for some families.

You will work in partnership with families and other agencies to identify what is needed to meet desired outcomes for children/young people (0-25) with SEN. This will include home visits to families and attendance at Annual Reviews and multi-agency meetings as appropriate. Collect and process information and specialist advice to inform the assessment and Education, Heath & Care Plan (EHCP)

Arrange a child or young person’s placement and any additional services, in collaboration with parents/carers and colleagues in other agencies.

You will work as part of a small team covering a geographical area of West Sussex under the direction of a Team Manager and with the support of a Senior Planning Coordinator. You will have excellent communication skills, including active listening skills and the ability to work sensitively with children, young people and their families in a problem-solving task-oriented way. You will be able to encourage engagement and participation and will coordinate and facilitate meetings with families and other agencies. Accurate recording and a commitment to working in sometimes stressful situations are essential qualities for the job.
